The Division of Student Affairs is committed to creating a safe and supportive learning community at 十大彩票平台. We depend on members of the college community to identify and report behaviors of concern so that we can provide students in distress with appropriate support services and resources. Please use the forms below to complete a report. If you have any questions, please email Student Affairs at student_affairs@gmbot.net.

Use this form to report potential violations of the Student Code of Conduct for follow-up by Student Affairs.

Share-A-Concern

Use this form to report a concern for the general health and well-being of a student. This report will be sent to the Behavioral Intervention Team (BIT).

Policy on Affirmative Action (PAA) and Title IX Concerns

The general 十大彩票平台 community and non-confidential employees should use this form to report concerns they have for a student who may be experiencing discrimination that violates the College’s Policy on Affirmative Action, Equal Opportunity & Diversity (PAA). This form should also be used to report concerns related to parental status, pregnancy, or pregnancy related conditions.

You do not need to determine if the behavior is Prohibited Conduct or Sex-Based Harassment, nor do you have to explain if it falls within the PAA definition of Parental Status, Pregnancy or Related Condition. Instead, upon receiving the disclosure just complete the form which will go directly to Dr. Kathryn C. Senie, Director of Compliance who serves as the College’s Affirmative Action Officer, Title IX Coordinator/ADA Coordinator.
